Medical Form Note

In accordance with New York City’s Department of Health guidelines, each student must submit a new medical form before the start of each school year. New Student forms must be dated after 7/31/24. Returning Student forms expire after one year. This means a form signed and dated by your child’s doctor on 3/1/24 would be valid through 3/1/25. Contact Sonia with questions.
