Medical Form Note

Per NYC Department of Health regulations, all students must have a current medical form on file, including documentation of all required immunizations.

Medical forms are valid for one year from the date of the physical exam. You are required to submit an updated medical form for your child after their annual visit.

For New Students: Please submit your child’s most recent medical form.
For Returning Students: If your child’s medical form was not updated during the school year, please submit their most recent annual physical.
